One question, and the standard answers an inspector can pick for it. The answers are the part that matters: a picked observation sets the score and pre-writes the recommendation, so the same finding reads the same way at every site.

The question

Write it so the answer is a judgement about the site, not about the inspector's mood.

Shown inline on the form. An inspector guessing the standard produces inconsistent data.

Capitals, numbers and underscores. Never reused.

Standard observations

The answers an inspector picks. Include the good state as well as the problems.
